Patient Forms and Insurance

Please fill out and bring the following forms to your appointment along with your:

  • Medical and vision insurance cards or account information
  • Current glasses prescription or contact lens prescription if available


Although we strive to know all your benefits prior to providing care, sometimes, all we can verify is that your medical insurance and/or managed vision care benefits are active. Rarely, we cannot even verify active coverage. We strongly encourage you to call your medical insurance and vision carrier(s) and familiarize yourself with your benefits.
